Our Treatment Programs
A full continuum of evidence-based addiction treatment at our Livermore, California facility — from medical detox through long-term aftercare.
Treatment Options at MTR Rehab
MTR Rehab in Livermore, California offers a structured continuum of care. Each program is tailored to the clinical needs of the individual, with licensed medical and therapeutic staff guiding every step of the recovery process.
Medical Detox Program
Medical detoxification is the first and most critical step in the treatment of physical drug or alcohol dependence. When the body has developed a physiological reliance on a substance, abrupt cessation can trigger potentially dangerous withdrawal symptoms. At MTR Rehab, detox is never managed alone — every patient is under continuous clinical oversight from the moment they arrive.
Our detox unit in Livermore is staffed around the clock by physicians, nurses, and addiction counselors who monitor vital signs, manage withdrawal symptoms, and adjust medication protocols as needed. The goal is to move each patient through the acute withdrawal phase as safely and comfortably as possible, then transition them into the appropriate level of continuing care.
- 24/7 medical monitoring by licensed clinical staff
- Medication-Assisted Treatment (MAT) where clinically indicated
- Individual clinical assessments at intake and throughout detox
- Withdrawal symptom management with evidence-based protocols
- Psychiatric evaluation available on site
- Seamless transition planning into residential or outpatient care
Our Detox Process
MTR Rehab follows a structured, three-phase detoxification process designed to stabilize the patient medically and prepare them for ongoing addiction treatment.
Assessment
Upon admission, our clinical team conducts a thorough medical and psychiatric evaluation. We document substance use history, physical health status, any co-occurring conditions, and previous treatment episodes to create an individualized detox plan.
Stabilization
Our physicians oversee the medically managed withdrawal process. Vital signs are monitored continuously. FDA-approved medications may be administered to reduce withdrawal severity and minimize the risk of complications such as seizures or severe dehydration.
Transition to Treatment
As medical stability is achieved, our care coordinators work with each patient to plan the next step — whether that is residential treatment, PHP, or another level of care appropriate to their clinical needs and circumstances.
Substances We Treat in Detox
MTR Rehab provides medically supervised detoxification for a wide range of substance dependencies. All detox admissions are evaluated individually to determine the safest withdrawal protocol.
Alcohol
Alcohol withdrawal can be life-threatening, particularly for long-term heavy drinkers. Our team manages benzodiazepine-assisted alcohol detox protocols to prevent seizures and delirium tremens (DTs).
Opioids, Heroin & Fentanyl
Opioid withdrawal, while rarely fatal on its own, produces intense physical discomfort. MAT with buprenorphine or methadone is available when clinically indicated to reduce withdrawal severity and cravings.
Benzodiazepines
Benzodiazepine withdrawal requires careful medical tapering. Abrupt cessation can lead to seizures. Our physicians manage individualized tapering schedules in a medically controlled setting.
Cocaine & Methamphetamine
Stimulant withdrawal is primarily psychological — characterized by fatigue, depression, and intense cravings. Our clinical team provides psychiatric support and symptom management throughout this phase.
Prescription Drugs
Dependence on prescription opioids, sedatives, or stimulants requires the same careful medical management as illicit substance withdrawal. Our clinicians develop individualized protocols based on the specific medication and pattern of use.
Marijuana
Heavy, long-term cannabis use can produce a withdrawal syndrome marked by irritability, sleep disruption, and appetite changes. Our team provides supportive care and symptom management during this transition period.
Detox duration is typically 3 to 7 days depending on the substance, severity of dependence, and individual health factors. All admissions include a clinical assessment to determine the appropriate protocol.
Residential Treatment Program
Residential treatment at MTR Rehab provides a structured, immersive therapeutic environment for individuals who require 24-hour support during the early stages of recovery. Patients reside at our Livermore facility for the duration of their program, removing them from the triggers, stressors, and environments that have sustained their addiction.
Residential care is appropriate for individuals who have completed medical detox, those with a history of relapse following outpatient treatment, and those whose home environment does not support early recovery. It is also the recommended level of care for individuals with more severe or long-standing substance use disorders.
- Individual therapy with a dedicated licensed counselor
- Daily group therapy sessions using CBT and DBT approaches
- Family therapy and structured family involvement
- 12-Step facilitation and peer support
- Holistic therapies integrated into weekly programming
- Medical oversight and psychiatric services on site
- Discharge planning and aftercare coordination

Program Length Options
Research consistently shows that longer residential treatment stays correlate with improved long-term outcomes. MTR Rehab offers three program length options to accommodate varying clinical needs.
30-Day Program
Our 30-day residential program provides intensive, focused treatment for individuals with moderate substance use disorders or those who have completed detox and need a concentrated period of structured care. This option is appropriate for individuals with strong family and social support and a lower risk of relapse.
60-Day Program
The 60-day program allows deeper engagement with therapeutic modalities and more time to address underlying behavioral patterns. Patients work on relapse prevention skills, family relationships, and life skills that support long-term recovery. This is our most commonly recommended length of stay.
90-Day Program
Our 90-day program is designed for individuals with severe substance use disorders, multiple prior treatment episodes, or significant co-occurring mental health concerns. Extended residential care provides the time needed to establish stable recovery habits, build a solid support network, and prepare for a successful return to daily life.
All residential placements begin with a comprehensive clinical assessment. Our treatment team will recommend the appropriate length of stay based on your individual clinical profile.
Start the Admissions ProcessOutpatient Treatment Programs
MTR Rehab's outpatient programs provide clinical structure and therapeutic support while allowing patients to live at home and, in many cases, maintain employment or family responsibilities. Our outpatient services are available both as a primary level of care for individuals with milder presentations and as step-down care following residential treatment.
All outpatient programming takes place at our Livermore facility and is delivered by the same licensed clinical team that staffs our inpatient services. Group therapy sessions, individual counseling, medication management, and family programming are integrated across all outpatient levels of care.
- Evidence-based group and individual therapy
- Medication-Assisted Treatment (MAT) available
- Flexible scheduling accommodating work and family
- Drug and alcohol testing as clinically indicated
- Family therapy and education
- Coordination with residential discharge planning
- Alumni and aftercare support
Levels of Outpatient Care
Our outpatient continuum follows the ASAM (American Society of Addiction Medicine) criteria and is structured into three levels. Patients progress through these levels as clinical stability improves.
Partial Hospitalization Program (PHP)
Duration: 2 to 4 weeks
PHP is the most intensive outpatient level of care, designed as an immediate step down from residential treatment or as a primary level of care for individuals who require structured daily programming but do not need 24-hour monitoring.
Patients attend programming five days per week, six or more hours per day, engaging in group therapy, individual counseling, medication management, and psychiatric services. PHP provides near-residential levels of clinical intensity while allowing patients to return home in the evenings.
- Five days per week, approximately six hours per day
- Daily group therapy and psychoeducation
- Weekly individual counseling sessions
- Psychiatric evaluation and medication management
- Structured relapse prevention curriculum
Intensive Outpatient Program (IOP)
Duration: 8 to 12 weeks
IOP offers a meaningful level of clinical structure while accommodating greater independence. Patients attend sessions three days per week, three hours per session, allowing for work or school during the remainder of the day.
IOP programming focuses on relapse prevention, coping skills development, peer support, and family engagement. It is well-suited for individuals transitioning out of PHP, those completing residential treatment with stable recovery support, or those whose substance use disorder does not require higher-intensity placement.
- Three days per week, three hours per session
- Group therapy: CBT, DBT, and 12-Step facilitation
- Bi-weekly individual counseling
- Family therapy available
- Ongoing drug testing and accountability
Standard Outpatient Program
Duration: Ongoing, as clinically indicated
Standard outpatient is the least restrictive level of formal addiction treatment. It typically involves one to two sessions per week — individual counseling, group therapy, or a combination — and is appropriate for individuals with strong recovery foundations who need continued professional support to maintain sobriety.
Standard outpatient also serves as a continuing care option for individuals who have completed IOP and wish to maintain a therapeutic relationship with MTR Rehab's clinical team during ongoing recovery.
- One to two sessions per week
- Individual and group therapy options
- Medication management coordination
- Alumni check-ins and support
Step-Down Care: The Continuum in Practice
Recovery is not a single event — it is a process. MTR Rehab's continuum of care is designed so that each level flows naturally into the next, reducing the risk of gaps in support during the critical transition periods that most commonly precede relapse.
Medical Detox
Physical stabilization under 24/7 medical oversight. The foundation before any therapeutic work begins.
Residential Treatment
Immersive inpatient rehab with structured daily programming for 30, 60, or 90 days.
Partial Hospitalization (PHP)
Five days per week of intensive outpatient programming. Near-residential intensity with home sleeping.
Intensive Outpatient (IOP)
Three days per week of structured group and individual therapy. Enables work and family re-engagement.
Outpatient & Aftercare
Ongoing individual counseling, alumni support, and community accountability to sustain long-term recovery.
Dual Diagnosis Treatment
Dual diagnosis — also referred to as co-occurring disorders — describes the presence of both a substance use disorder and one or more mental health conditions in the same individual. Research indicates that the majority of people seeking addiction treatment have at least one co-occurring psychiatric diagnosis. When only the addiction is treated while the underlying mental health condition goes unaddressed, the risk of relapse remains high.
At MTR Rehab, dual diagnosis is not an add-on service — it is a core component of our clinical model. Our licensed staff includes mental health clinicians and psychiatrists who collaborate with addiction counselors to deliver integrated treatment from the first day of admission through discharge and aftercare planning.
- Integrated psychiatric and addiction treatment under one roof
- Comprehensive psychiatric evaluation at intake
- Medication management by board-certified psychiatrists
- Trauma-informed therapy modalities (DBT, CPT, EMDR-informed)
- Concurrent treatment for both conditions throughout the stay
- Coordinated discharge planning with outpatient mental health providers
Common Co-Occurring Disorders
MTR Rehab has experience treating a broad range of mental health conditions that frequently present alongside substance use disorders. The following are among the most common presentations we treat at our Livermore facility.
Depression
Major depressive disorder is one of the most prevalent co-occurring conditions in individuals with addiction. Substances are often used initially to self-medicate depressive symptoms, creating a cycle of dependence that deepens both conditions. MTR Rehab's integrated care addresses depression through medication management and evidence-based therapy while simultaneously treating the substance use disorder.
Anxiety Disorders
Generalized anxiety, panic disorder, social anxiety, and other anxiety presentations frequently co-occur with substance use, particularly alcohol and benzodiazepine dependence. Our clinical team identifies anxiety contributors, establishes appropriate pharmacotherapy where indicated, and teaches evidence-based coping strategies through CBT and DBT groups.
Post-Traumatic Stress Disorder (PTSD)
Trauma and addiction are closely linked. Many individuals entering treatment carry unresolved traumatic experiences that have driven or worsened substance use. Our trauma-informed clinical framework ensures that PTSD is identified and treated alongside the addiction without retraumatization, using evidence-based approaches suitable for the appropriate stage of recovery.
Bipolar Disorder
Bipolar disorder's mood instability — cycling between depressive lows and manic or hypomanic highs — is strongly associated with substance misuse. Substance use often begins as an attempt to regulate mood episodes. Accurate diagnosis requires a period of sobriety to distinguish substance-induced mood symptoms from a primary bipolar diagnosis. MTR Rehab's psychiatrists are experienced in this complex differential diagnostic process.
Our Integrated Treatment Approach
Effective dual diagnosis care requires that both conditions be treated simultaneously by a coordinated clinical team — not sequentially or in separate programs.
Comprehensive Intake Assessment
Every patient receives a psychiatric evaluation, substance use history review, and medical assessment within the first 24 hours of admission. This informs an individualized treatment plan that addresses both diagnoses from day one.
Integrated Therapy Groups
Our group therapy curriculum includes sessions specifically designed for dual diagnosis patients — covering the relationship between mental health and addiction, emotional regulation, coping with psychiatric symptoms without substances, and building healthy behavioral patterns.
Medication Management
Board-certified psychiatrists oversee psychiatric medication evaluations and ongoing management throughout treatment. Medication protocols are reviewed regularly and adjusted based on clinical response, with attention to medications' interaction with substances and recovery goals.
Coordinated Discharge Planning
Patients completing their program receive a discharge plan that addresses both the addiction and the mental health diagnosis — including outpatient therapy referrals, psychiatric follow-up, medication plans, and community support connections to support continued recovery in the Livermore area and beyond.
If you or a loved one is struggling with both a mental health condition and a substance use disorder, MTR Rehab's integrated dual diagnosis program may be the right fit. Contact our admissions team to discuss your situation.
